Sussex Integrated Dementia Care and End of Life Pathway

This resource pack has been adopted as best practice guidance. It identifies the key actions to be taken by practitioners at each phase to improve end of life care for people with dementia, their relatives, carers and significant others.


About this resource pack

This pathway consists of six phases:

  • Recognising there is a problem
  • Discovering that the condition is dementia
  • Living well with dementia
  • Getting the right help at the right time
  • Nearing the end of life, including the last few days of life
  • Care for carers and relatives after death

The resource pack contains links from each phase to research, policy, guidance and information. Although the pack is aimed at practitioners, some links will take you to material that can be shared with people with dementia, their carers and their relatives.
