This privacy notice covers use of our online ‘Adult Social Care & Health factsheet and information leaflet order’ form.

What information is being used?

In order to provide the service we will collect your name, your email address, phone number, the organisation that you work for and the address to which we will be sending the factsheets or information leaflets.

How will your information be used?

We will use your name to record who has made the request. Your contact details will only be used in order to follow-up on your request. We will need the address(es) of premises in order to dispatch leaflets and factsheets to the correct location(s).

We aim to maintain high standards, adopt best practice for our record keeping and regularly check and report on how we are doing. Your information is never sold for direct marketing purposes.

Our staff are trained to handle your information correctly and protect your confidentiality and privacy.

Your information is not processed outside of the European Economic Area.

Information will be retained according to the retention period applicable to your service request.
