If the death is registered at an East Sussex County Council registrar you will be able to inform the registrar service as part of the appointment.

Please enclose:

• a photocopy of the death certificate (if available)
• if you have it, the bus pass (cut in two).

If you cannot print the form, please confirm the your details in a letter and send it with your photocopied proof to Transport Hub – Public Services, East Sussex County Council, County Hall, St Anne’s Crescent, Lewes, BN7 1UE.
