The postcode lookup on some of our online forms is currently unavailable. This affects online forms for Subject Access Requests, Freedom of Information requests and complaints, if you are adding a postal address.
You can still add an address manually. Sorry for any inconvenience caused. We are working to fix it and hope to restore the lookup service soon.

Read our guidance on making a subject access request before making your request. Documents can be attached before you submit the form.
Please provide as much detail as possible to help us find the information you need. If we need to clarify your request, the time taken to do this may change the deadline to respond.
Once we have validated your request, we aim to respond to you within one month. Complex requests may take longer, and we will let you know within one month if this is the case. Some information may be withheld, and the reasons will be explained to you. If we do not hold the information you have requested, we will let you know as soon as possible.
