This form is for you if you, or someone you care about, is struggling with alcohol or any drug and you wish to contact the U25’s Substance Misuse Service.

We offer free, flexible and confidential help and support services across the county.

How addictions can affect you

You may be:

  • spending too much money on cannabis, alcohol or other substances
  • tired and anxious
  • missing school, college or work
  • falling out with family or friends
  • getting into trouble with the police.

How the East Sussex U25’s Substance Misuse Service works with you

The Under 25’s Substance Misuse Service is an integrated multidisciplinary team led by the local authority. It works across the whole of East Sussex from Children’s Services teams and other accessible young person-centred locations.

The service offers Tier 4 specialist drug treatment provision.

There are staff in the team from different professional backgrounds like nurses and social workers all working together to assist you.

U25’s Substance Misuse Service: your information and you

To ensure you can get the right support, we require some basic information about you:

  • to understand how you feel we can help you
  • to develop a treatment plan with you to help you make the changes you want
  • to deliver substance misuse services across East Sussex

This may include asking for:

  • personal information: name, address, contact information, date of birth, and other identifying information
  • information about you and your family
  • information about your concerns: this may include information about the specific concerns you have about your substance misuse and its impact on you for example, employment, education, physical and/or mental health
  • information about your preferences: this may include information about the specific services you would prefer to receive and where you would like to receive them.

To deliver our work with you we may also share some information about you and your family with other professionals. We may also ask them to share information with us. This helps us understand what assistance you may need. By professionals, we mean other services that may have worked with you and your family or may provide advice and support in the future.

Only staff who need it to do their job can look at any personal information you provide. They might need your information to arrange a home visit or to arrange to meet you outside of your home. These staff have completed training on the proper use of your data and on confidentiality.

Sometimes we share or ask for information to help improve our services. Information shared within the Council to improve our service would be anonymised.
