The Transitions Service supports young people from the ages of 16-25 with severe and enduring disabilities and their families in a planned and personalised way.

We have a great opportunity for a motivated and proactive registered Social Worker who is committed to ensuring that the service supports young people and their families to make choices about their future in an individual and timely manner.   

To fully support young people as they transition into ASC we work closely with Children’s Disability Social Care, Education and Health Services. Knowledge, experience, and an understanding in this area of social work would be advantageous as will experience of working with young people with disabilities and their families. 

If you don’t already have this breadth of experience, we can train you. The Transition service is part of a large Council which actively promotes social workers CPD by offering regular training, appraisals, and supervision.   

Our hybrid working arrangements allow for a mix of being home and office based throughout the week. This post covers clients living across the county.    

We are looking for candidates who:

  • Are keen to develop further as a social worker.
  • Can work as part of a team.
  • Have good communication skills and feeling confident in discussions with other  professional disciplines.
  • Have an ability to meet travel requirements of the post.
  • Have a commitment to training and personal development.

Working Pattern: Monday-Thursday 8:30am-5:00pm and Friday 08:30am-4:30pm.

Your starting salary will be pro rata if the above position is part-time or term-time only.

Work Permits: we may be able to obtain a Work Permit for this post but this is subject to meeting the requirements of the UK Border Agency’s Points-based Immigration System.
