Consent required
We can only help with complaints from people who do not have parental responsibility (PR) if we have the right consent. This means we need consent from the young person who uses our service or from their parent or carer, who has parental responsibility.
To help us help you, complete our consent form and attach it to your complaint. If we do not get consent, we will not be able to respond.

- CSD General Feedback (email us). I’d like to give general feedback. I realise I may not get a formal response
