Apply for an on-street disabled parking bay

Please read, before completing the form.

Eligibility

Provision of an on-street bay is not automatic. Applicants must:

  • hold a valid blue badge
  • have a permanent and substantial disability, which means being unable to walk, or have considerable difficulty in walking any distance.
  • not have a suitable off-street area available to park
  • have a vehicle that is registered at their address.

Documents for uploading

Make sure you have the relevant documents saved as scans or photos before you start. Only these specific documents can be accepted as proof of eligibility:

Vehicle ownership

  • Vehicle registration document (V5C) or
  • Motability Operations Hire Agreement

Disability Living Allowance documents

If you receive the higher/enhanced rate mobility component of the Disability Living Allowance (DLA), please provide a copy of either:

  • Disability Living Allowance confirmation letter or
  • Personal Independence Payment confirmation letter or Statement of Entitlement

Attendance Allowance

If you receive Attendance Allowance, please provide a copy of your confirmation letter or Statement of Entitlement.

Please also note

  • Disabled bays can be used by any vehicle displaying a valid blue badge.
  • We may need to contact your doctor about your application.

Application form

Section 1: Your details (details of the Blue Badge holder)

Full address (including house number or name) of the property that the bay is being requested for.




Please make sure you have provided your email address.

Section 2: Your vehicle (details of the Blue Badge holder's vehicle)



Please note: if the vehicle you use is not registered at your address, you will not qualify for a disabled parking bay.

If your Blue Badge is due to expire in less than 6 weeks from the date you submit this form, your application will not be accepted.

Section 4: Your benefits



If you receive the higher/enhanced rate, you will need to provide proof of your entitlement (see Section 8 - Proof of eligibility).




If you receive Attendance Allowance, you will need to provide proof of your entitlement (see Section 8 - Proof of eligibility).

Please note: your application will automatically be sent through to the Adult Social Care Team if it is considered that a mobility assessment is necessary, after the location has been assessed. You will be advised if this has been done.
