Based on your answers, you need a paper copy of our consent form.

You will need to fill it in and post it back to us with a copy of the photo ID of the person you are representing.

Once you have done this, come back to this page to continue making your complaint. You may want to bookmark this page for later use.

If you don't have a signed consent form, you can still give us general feedback, but we might not be able to reply to you.

Your next steps

1. Email CSD general feedback with your address so we can send you a consent form. Tell us who you are and your relationship to the person you are representing.
2. Fill in the form with the person you are representing.
3. Send the form back to us with a copy of their photo ID.
