Based on your answers, you need a paper copy of our consent form.

You must fill in this form and send it to us with a copy of the photo ID of the person you are helping.

After you have done this, come back to this page to finish making your complaint. You might want to save this page so you can find it easily later.

If you don't have a signed consent form, you can still give us general feedback, but we might not be able to reply to you.

Your next steps

1. Email CSD general feedback with your address so we can send you a consent form. Say who you are and how you know the person you are helping.
2. Fill in the form with the person you are representing.
3. Send the form back to us with a copy of their photo ID.
