Use the Subject Access Request (SAR) form below if you want to access any personal information that the Council holds for you or someone that you are responsible for. The form helps us locate the information, but they are not compulsory, and there are alternative instructions below for getting in contact.

Please fill in the web form below and upload the following documents:

  • The SAR pdf or Microsoft Word document
  • Two pieces of identification to prove who you are. Photocopies of original documents are acceptable.
  • Proof of entitlement to make the request. This is only if you are making the request for information about someone other than yourself (please see guidance notes on the SAR form).

Once we have received your request and the required identification, we aim to respond to you within one month. Complex Requests may take longer and we will let you know within one month if this is the case. Some information may be withheld, and the reasons will be explained to you.
