Please fill in one of the forms below. This is not required, but it helps us locate your information. If we need further information, the response time will change to include the time taken to clarify your request, so please respond clearly and with as much detail as possible.

Complete the web form and upload the following documents:

• Completed form or letter detailing the information you are requesting
• Relevant supporting evidence (guidance is on the form). Photocopies of original documents are acceptable.
• If you are a current employee, please let us know and see the guidance notes on the form

Once received we aim to respond to you within one month. Complex requests may take longer, and we will let you know within one month if this is the case. Some information may be withheld, and the reasons will be explained to you. If we do not hold the information you have requested, we will let you know as soon as possible.
