Do you struggle with mobility or carrying out daily living activities such as preparing food? We may be able to help you learn or relearn skills to help you to be more independent.

This is often called reablement or rehabilitation.

Joint Community Rehabilitation team (JCR)

Our JCR team can help you regain your strength and mobility at home.

They can:

  • help you get support from occupational therapists
  • give you access to physiotherapy
  • make sure you have the right equipment in your home to help you live independently.

You’ll need to be referred by your GP or a health or social care professional.
