The Adult Social Care Learning Disability Assessment and Care Management Service has an exciting new opportunity for a Senior Practitioner / Complex Case Worker to join us for a two-year period. The role is focused on the Learning Disability and Autism Programme which aims to reduce the number of people with autism and learning disability and/or mental health in England who are in hospital, and instead provide their care in the community and closer to home.  Your main role will focus on managing hospital discharges back  into the community, working with commissioning colleagues, partners from health and provider organisations to meet identified outcomes. 

We are looking for someone who wants to make a real difference to the lives of people who have been in hospital. You will be self-motivated, able to work alone whilst also forming productive partnerships with colleagues, able to demonstrate a strong sense of initiative, adaptable and resilient with a commitment to the social model.

This is a unique role which offers the opportunity to work in partnership across organisations and professions. You will be working across Learning Disability Services, Mental Health, Forensic, inpatient and community services.  You will be part of a wider LD team and have access to supervision and support on an ongoing basis.  

The ability to communicate clearly will be a core component of this role and you will need to draw upon your experience of how to navigate complex care pathways with the support of experienced colleagues. Specialist assessments and robust risk management plans will be central to the role.

You should have an understanding of the Learning Disability and Autism Agenda and experience and a working knowledge of the MHA. 

This post is fundamental to the success of the LDA programme in East Sussex, it represents an investment in supporting people to develop a good and meaningful life. It is an opportunity to put Social Work values into action and whilst this is a challenging role, the post holder will receive a high level of support. 

We have offices located in Hastings and Uckfield and your contracted base will be discussed at interview. Interview will take place via MS Teams within 2 weeks of the closing date.

Your starting salary will be pro rata if the above position is part-time or term-time only.
