Item 5a - Fit for the Future - Proposals for change to NHS services in East Sussex

To receive proposals from East Sussex Downs and Weald PCT and Hastings and Rother PCT for changes in the provision of NHS services in East Sussex

Item 5b - Fit for the Future - Public consultation process

To receive and comment on the Primary Care Trusts' (PCTs') plan for undertaking public consultation (appendix 1), having explored the themes outlined in section 3

Item 5c - Fit for the Future - scrutiny process

To agree an appropriate evidence gathering and scrutiny process which will enable HOSC to respond to the PCT's proposals under Fit for the Future

Item 5d - Fit for the Future in West Sussex and Brighton and Hove

To update HOSC on the progress of 'Fit for the Future' in West Sussex and Brighton & Hove. To consider whether forthcoming proposals for changes to NHS services in West Sussex are likely to constitute a susbstantial variation to services for East Sussex residents and therefore require HOSC to participate in a joint HOSC to examine these proposals

Item 6 - Delayed Transfers of Care - update

To consider the latest position on Delayed Transfers of Care and the initiatives in place to reduce these

Item 7 - Best Care Best Place - update

To update HOSC on the progress of the joint review with Brighton & Hove City Council and West Sussex County Council on health service plans under the 'Best Care Best Place' banner

Item 8 - Review of services for young people with long term conditions - update

To consider the way forward for the review following the PCT's establishment of the Long Term Conditions Steering Group which will build on the legacy of the Transforming Chronic Care Programme

Item 9 - Healthcare Commission Annual Health Check process 2006/07

To update HOSC on the annual health check process for 2006/07 and to agree HOSC's approach to submitting commentary on the performance of NHS organisations against the national standards

Item 10 - HOSC relationship with Patient and Public Involvement Forums (PPIF's) and development of Local Involvement Networks (LINks)

To update HOSC on the progress nationally and locally towards the establishment of LINks. To review the HOSC relationship with East Sussex PPIFs and recommend how this can develop in anticipation of the establishment of an East Sussex LINk
