Blue Badge holders can use this form to apply to have a disabled parking bay outside their home.

The notes at the end of the form describe who is entitled to apply.

Guidance notes

If you receive the the mobility component (higher rate) of the disability allowance, you will need to send us a copy of your entitlement. You can use the 'Print, write and post' version of the form and send it to us with your entitlement. If you would prefer to send us the form by email, please post a copy of your entitlement to:
