Applying for adult social care services
There are a number of simple steps to apply for adult services:
- Apply online for services for adults
- Firstly, complete the online application form.
- Our Social Care Direct team will contact you
- Secondly, after receiving your application form we will contact you to discuss your needs. If you need to speak to someone please telephone Social Care Direct on 0345 60 80 191.
- Completing the assessment process
- Finally, when we contact you, we will arrange for your needs to be fully assessed by a member of staff. At the end of this assessment process we will write a report about you and your needs.
