Rehabilitation
What is rehabilitation?
Rehabilitation can help you to develop and improve skills to enable you to continue living at home without the need for long-term residential care. You can download further information in our booklets.
A rehabilitation break is short-term, usually not longer than 12 weeks.
Who is it for?
Rehabilitation is available to older people who are 65 or over. If you are determined to stay in your own home rather than going into residential care, and prepared to work hard to keep your independence, then this type of care may be for you.
It can also help you if you need to leave hospital because you no longer need in-patient care but are unable to return home alone.
What types of breaks are there?
There are two main types of rehabilitation:
Care when you leave hospital
If you are in hospital but no longer need medical care, the staff there will refer you to us if you are unable to return home alone.
You will be admitted to one of our care centres where occupational therapists and physiotherapists will assess your needs. They will work out a programme of activities to help you. Many people can return home after two weeks of care, often with some further home care support.
Longer term rehabilitation
Some people need to receive longer term rehabilitation. The first phase of longer term rehabilitation can last up to six weeks. This will happen in one of our chosen residential homes. The same person will help co-ordinate your rehabilitation programme through this time.
Occupational therapists and physiotherapists will look at your needs and work out a programme of activities to help develop your daily living skills. The residential home staff will help you to achieve the goals you have talked about.
After this residential phase you go back to your own home and can have up to six weeks of back-up support from our home-care services. We might also be able to arrange other specialist help during this time. At the end of the programme we will look again at what support you might still need for the future.
How much will it cost?
We will ask you to pay for or contribute towards the cost of your residential care and home care. The amount you pay depends on how much you can afford and is worked out by through a financial assessment. Find out more about paying for services
How can I apply?
To apply for rehabilitation care you have to receive an assessment of your needs. This is described in our page about applying for services. The money available limits the services that we can offer, and we cannot meet everyone’s needs all the time. Some people will have more important needs than others and the assessment process helps us find out whose needs are greatest.
