Early leaver claim form (LGPS-2A)

If you decide to leave the pension scheme before you retire, you have three choices:

  • take a refund of contributions
  • transfer your pension to another scheme
  • leave the benefits you have built up and take them as a deferred pension when you retire.

Complete this form to show what you want to do.

If you have not already done so, you will also need to complete Part B of the Membership questionnaire (LGPS-1) to show that you want to leave the pension scheme.
