Visual impairment referral – applying for support for your child

Children with significant visual impairment can get help at home and school. This can include regular school and home visits by a specialist teacher of the visually impaired, and provision of special equipment.

What is a significant visual impairment?

Your child may be considered to have a significant visual impairment if for example they are currently being seen by a consultant opthalmologist.

How to apply

Please complete the referral form and return it to the address below. Anyone can fill in the form, including practitioners, nurses or teachers working with the child, but the parents or carers must be told about the referral.

Return your form to the Sensory Needs Team.

We will contact you within five working days of receiving the referral form to confirm if the child is entitled to receive support. A specialist teacher of the visually impaired will visit the child at home or school to assess their needs.
