How to apply for children and family services

This form is for anyone who wants the assistance of our children and family services. Complete this form if you have a concern about a child or young person, you need help with your own child, or you are a young person wanting help.

• Once you have completed this form, please post it to your local Duty and Assessment team.
• Once they receive the form, the team will telephone you to discuss your request.
