Applying for children and family services

If you have a concern about a child or young person, you need help with your own child, or you are a young person wanting help, you can apply for help from our children and family services.

There are two steps to requesting children and family services.

Download and return the application form. After receiving your form we will get in touch to discuss your situation.

Step two – Children and family assessment

As part of these discussions we will meet with you to produce a written assessment of your needs.
